Subacute Rehab

Short-term, intensive therapy in a skilled-nursing setting after a hospital stay.

What they don't tell you

The hospital discharges to a SNF for rehab. Medicare pays. Therapy is 1-3 hours a day. The goal is to recover enough function to go home. About 30% of admissions never make it home — they convert to long-term residents. Ask, on day one, what the plan to home looks like.
