Advance Directive

A written document spelling out your healthcare wishes if you can't speak for yourself.

What they don't tell you

Two parts: a living will (what care you want) and a durable power of attorney for healthcare (who decides). Free templates exist; lawyers aren't required, but the document needs to follow your state's witness or notary rules. Useless if no one knows where you keep it.
